What this support includes:
Clinical Health Support
Skilled nursing services such as wound management, medication administration, continence support, and monitoring of chronic conditions including diabetes, epilepsy, and respiratory illnesses.
Post-Surgical & Rehabilitation Care
Support with recovery following hospital stays or surgery, ensuring participants regain independence and confidence during their rehabilitation journey.
Palliative and Complex Care
Compassionate, person-centred care for participants with life-limiting conditions, focusing on dignity, comfort, and wellbeing.
Health Education & Empowerment
Guidance and training for participants and families to better understand health conditions, medication routines, and ongoing management strategies.
Care Coordination
Collaboration with doctors, allied health professionals, and support workers to ensure continuity of care and a holistic approach to each participant’s wellbeing.
